That's the reason why there has been controversy surrounding the issue on how accurate their measurement is. Let's say, if site A is getting 100 visitors per day and 80% of them are using Alexa compared to site B which is getting 1000 visitors per day but only 5% are using Alexa, this means that the ranking of site A is much higher in Alexa measurement.
Although one may argue that this scenario is rather unlikely, it could still be possible if site A is a webmasters forum whereby the visitors all have strong webmasters related knowledge and knowing how to use Alexa toolbar.
